The Material Content Declaration Form workbook is comprised of many tabs for information and reference, data collection, administration, utilities, and storing important information and help.
After importing the Tab-Delimited text file provided by Lexmark with the Material Declaration request, some fields will be pre-populated.
The button displays a Help screen for the current tab, including a screen shot with instructions for completing the worksheet.
The button displays an information window for the currently selected column.
The Column Help window remains open and updates with information about the current column as you navigate throughout the workbook.
A Check button in each data entry tab validates the current data.
All previous tabs must be valid before a subsequent tab may be checked.
The Auto-Validation On/Off button toggles auto-validation for the workbook and is On by default.
An error message will appear whenever you try to navigate away from a tab with incomplete or invalid data.
We recommend working with Auto-Validation On.
The first three tabs of the Material Content Declaration Form workbook are informational worksheets, and are usually colored pink (some versions of Excel do not display different worksheet tab colors).

Table 3 Restricted Substances
Substances listed in this table are prohibited from use in parts or products supplied to Lexmark unless otherwise noted within the table. Any use of these substances (including unrestricted or exempted applications) must be declared to Lexmark.
Table 4 Reportable Substances
Any use of the substances listed on this table must be declared to Lexmark.
The next six tabs are data input tabs (usually colored blue) that you are required to complete, in order from left to right.
The worksheets are protected from being filled out if previous tabs have not passed validation checks.

General Information
This tab should be completed withe the imported, pre-populate data. This tab may not be edited.
Part Tree
Contains the Bill of Material for each saleable part (part supplied to Lexmark). The parts on this tab are pre-populated.
Parts with both sub-parts (BOM children) and materials should be given a separate sub-part as a 'container' for the material, with the parent part name plus an identifier.
Materials
This tab is used to define custom materials and list the substances that comprise them.
A custom material is any material used that is not listed on the Standard Materials tab.
Part Detail
This tab lists all of the BOM child parts that have no sub-parts, called 'leaf parts'.
The parts on this tab are pre-populated.
The material content information of each leaf part is entered here. The use of Lexmark Standard Materials is encouraged.
Standard Materials
Lexmark Standard Materials are pre-populated on this tab.
This tab is read only, to be used as a reference.
If a material used is not on this list, it must be added on the Materials tab.
Substance of Concern Detail
Fill in this tab for each saleable part that includes a material that contains any Substance of Concern in amounts above allowable thresholds.
The Administration tab is for Lexmark use only.
The Utilities tab is used to:
Import the requestor data from Lexmark. See Import Data Into Material Content Declaration Form.
Export workbook contents, without certifying.
Import Substance List, to update the list of substances.
Import RoHS Exemptions, to update the RoHS Exemptions information.
Tabs to the right of the Utilities tab are for Lexmark use only.
