The Material Content Declaration Form is used to collect Material Declaration information to import into ENOVIA.
The ENOVIA Material Content Declaration Form is an Excel spreadsheet called Compliance Connect.
The terms, Material Content Declaration and Compliance Connect form, may be used interchangeably.
The ENOVIA Compliance Connect form is programmed to:
Import Lexmark requestor data and pre-populate the form with parts to be reported.
Check and validate the part / material data.
Export certified data for sending to the requestor.
Microsoft Excel is required to use the Material Content Declaration / Compliance Connect form.
Macros import the pre-populate data, validate the data as you go, and certify and export the data.
The form requires that Excel macros are enabled.
If macros will not run, change the Microsoft Excel security settings (see Software Requirements for more information).
See Material Declaration Workbook Tabs for detailed information on the Material Content Declaration / Compliance Connect form workbook.
The Lexmark Material Content Declaration Form is available for download to anyone, regardless of ENOVIA access, at this address:

Double click the Material Declaration zip file to open it in Winzip, then Extract the form.

Depending on your computer screen size and resolution, it may be helpful to use the Excel Zoom control to reduce the size and fit more of the spreadsheet on your screen, or to increase the size for clearer viewing.
The Excel Zoom control is located on the View menu/tab and may also be shown as a slider bar on the lower right of the Excel window.
If you see only a blank screen, be sure to use the scroll bars to view the top left corner of the spreadsheet.
